Desktop case management provides the appropriate healthcare support for people with moderately complex or multiple injuries following a personal injury. A dedicated HCML rehabilitation case manager will support the injured person to achieve specific functional and vocational goals through a bespoke multidisciplinary rehabilitation programme.

Coordinated rehabilitation for moderate and multiple injuries

Where personal injury has affected someone’s quality of life or ability to do the things they did prior to the incident, a coordinated rehabilitation programme is required to help them recover as quickly as possible. Delays in assessment and treatment increase the likelihood that additional rehabilitation and more time will be needed for recovery.

Delivered via phone and video calls, desktop case management enables faster assessment and referral into the right treatment pathways.

With a dedicated rehabilitation case manager to develop and oversee a tailored multidisciplinary rehabilitation programme, it reduces unnecessary costs and waiting times, ensuring the person receives the most appropriate healthcare to suit their recovery needs.

Complete assessment

The rehabilitation case manager will complete an in-depth immediate needs assessment (INA) to identify all related symptoms and the impact on the rehabilitation client’s social and work life.

This will determine the person’s multidisciplinary treatment needs and the right level of case management support needed.

You’ll receive a comprehensive report with a detailed and fully costed rehabilitation plan.

Bespoke rehabilitation programme

Following assessment, the rehabilitation case manager will develop a clinically appropriate rehabilitation programme for the individual. They’ll oversee the rehabilitation client’s recovery, coordinating the relevant treatment, monitoring progress and supporting them virtually to achieve their rehabilitation goals.

In many cases, this includes supporting the person to remain in or return to work, and this is all managed by HCML.

The rehabilitation case manager will keep you updated on the client’s progress and ensure they are on track with their recovery, whilst monitoring costs.

The HCML Case Management team have clinical expertise across a variety of multidisciplinary backgrounds, including HCPC registered physiotherapists, psychologists, occupational therapists and registered general nurses.
