Case Management and Management Referrals
Our case management and management referral solutions give you and your employees guidance and independent practical advice so you can both manage the impact of health conditions.
Referral takes minutes through a bespoke portal.
Prior to assessment, we’ll get in touch with the referring HR or line manager to discuss specific concerns and any support that currently exists within the business to ensure we have the best possible understanding of the circumstances.
Dependent on the individual circumstances, the assessment can be carried out via phone, video call or in person.
Seeing the whole person
We don’t just look at conditions in isolation. We look at the whole person to identify specific health care needs and maximise the likelihood of a successful outcome. This includes symptoms, personal underlying factors, psychosocial factors, work and social life.
Solutions that work for you
Before issuing a comprehensive report, case managers will discuss the findings and recommendations with you, so you know what to expect. We work with the relevant managers and HR to consider possible adjustments that can be facilitated by the business to inform our recommendations, which can be supported through our case management services.
We conduct a post-assessment follow up as standard within 4 weeks of initial assessment. This is designed to support your employee with workplace attendance and performance, to provide progressive advice or signposting, and to capture key updates, such as return to work information or reassessment of clinical outcome measures.
Depending on condition we develop and implement return to work plans and can offer a range of interventions including counselling, CBT, physiotherapy, nutrition, psychosocial, sports medicine, executive coaching and wellbeing.
Empower employees
Where possible, we empower employees to take control of their own health by providing self-help, guided self-help with regular case manager contact, and signposting to other services such as:
- Careline: our Employee Assistance Programme (EAP)
- YouHalo: a 24/7 digital health and wellbeing advice and support service
- Physiotherapy: digital assessment, self-help and virtual programmes, and clinic booking facility
Capability Assessments & Equality Act 2010
Our priority is the employee’s return to health and wellbeing as soon as possible by providing a clinically led action plan to help you manage an individual’s return to work or performance.
On some occasions the role will no longer be suitable for the employee, and we will always be upfront and report directly on capability, motivation and recovery timeframes, including any implications under the Equality Act 2010.
