I’ve been referred to HCML
For rehabilitation clients following personal injury.
If you’ve been referred to HCML by a solicitor or insurer, you’re in safe hands. We’ll help guide you through the process of rehabilitation and recovery. Here’s what you can expect.
Your health and wellbeing are what matters to us. Experiencing an injury can have a huge impact on your quality of life. We’re here to support you throughout your recovery, from minor through to complex and life-changing injuries.
Serious and complex injuries
Our role is to help you through your rehabilitation journey, understanding your needs and ensuring you get the appropriate treatment and support. We work with the solicitors and insurers on your case, so you can be assured your needs are taken care of.
What you can expect:
Your own rehabilitation case manager
You’ll be assigned a dedicated HCML rehabilitation case manager who will support you throughout the rehabilitation process. Your case manager will be a qualified health professional with experience in cases like yours.
Immediate Needs Assessment
The first thing your rehabilitation case manager will do is conduct an ‘Immediate Needs Assessment’ (INA) with you. This can be face-to-face or over the phone. Your case manager will ask you about your injury, your symptoms and what you have experienced so far.
They’ll speak to any medical professionals who have been involved in your case to understand your medical history and any treatment you’ve already received or are currently receiving.
Your rehabilitation case manager will also discuss with you what your lifestyle was like prior to your injury. This includes what you do for work, what you enjoy doing socially and your responsibilities and commitments which may relate to your family or within the community.
With your consent, your rehabilitation case manager may speak to your family to help build a comprehensive picture of you and your lifestyle. This helps them to understand your goals and what you’d like to achieve.
If appropriate, they will also speak to your employer to discuss how your condition impacts on your ability to do your job and what adjustments may be needed in order to facilitate a return to work. In some cases, a return to your role may not be possible due to the nature of your injuries. In this instance your case manager will support you with finding alternative employment or other vocational roles such as charitable or voluntary work.
The more they understand about you, the better they’re able to support you.
From there, they will identify immediate priorities for your rehabilitation and set out a series of recommendations for treatment. This will be written up as a detailed report for your solicitor and, if applicable, the insurer.
Bespoke rehabilitation programme
Your rehabilitation case manager will create a bespoke programme for you, considering both short- and long-term goals, liaising with a range of stakeholders across private and statutory services to ensure you get the support you need. This can include therapy and rehabilitation, assistance with housing needs, support returning to work, transport and accessing the community.
They will work closely with you throughout your rehabilitation journey to:
- build a trusted relationship with you and your support network
- always be your advocate and communicate your best interests to those involved in your case
- ensure you are included in any decisions about your rehabilitation
- answer your questions, provide information and give you their full support
- review your progress against goals and adapt your programme as necessary
- help you achieve your goals as quickly and effectively as possible.
Moderate and non-complex injuries
If you have injuries including musculoskeletal injuries such as whiplash or back pain, and mental health concerns such as anxiety or depression, as a result of a personal injury, you may be supported by a rehabilitation case manager either via telephone or online.
What you can expect:
For multiple and moderate injuries, a HCML rehabilitation case manager will contact you to arrange an initial assessment over the phone or via video call.
During the initial assessment, the case manager will ask you to:
- explain the details of the accident/incident
- describe the injuries and symptoms you’re experiencing as a result of the incident – these could be physical and psychological
- share your medical history, medication and treatment you may have already received
- discuss the impact of your injuries on your work and social life.
The rehabilitation case manager will help you to define your rehabilitation goals and develop a bespoke rehabilitation programme that details recommended therapy, treatment and support.
They will put together a comprehensive report for the person that has referred you.
Once agreed with the referrer, your rehabilitation case manager will act as your first point of call – coordinating your treatment and therapy, reviewing your progress against agreed goals, and communicating with relevant stakeholders to ensure you get the right treatment at the right time.
