How the service works

Initial Contact

Once a referral is received, we make contact within 24 hours to arrange a triage assessment, helping people feel supported from the outset while ensuring timely access to the most appropriate support.

Holistic Triage Assessment

A clinical practitioner completes a biopsychosocial assessment, considering mental health, behaviour, lifestyle and wider context, to understand the full picture of what’s affecting each individual.

This includes:

  • Presenting concerns
  • History and wider context
  • Risk factors
  • Goals for support
  • The most appropriate treatment pathway

This ensures we understand the full picture – so support is matched to what each person actually needs.

Tailored Treatment Plan

Based on the assessment, we match each person with the right support level. This may include:

  • Guided self help
  • Counselling
  • Cognitive Behavioural Therapy (CBT)
  • Wellness and life coaching

By matching support to need, we avoid generic pathways and ensure more effective outcomes from the outset.

Case management and step-up support

Each individual is actively supported through a clinically led case management approach. Progress, outcomes and risk are continuously reviewed to ensure care remains aligned to need.

If circumstances change, support is stepped up or down to the most appropriate level, ensuring the right intensity of care at the right time.

This proactive approach helps maintain progress, prevent escalation and support a more sustainable return to work.

We provide high-quality, clinically informed reports at key points in the support journey, supporting Occupational Health decisions and authorisation of further treatment where needed.
