We provide clinically led case management that supports people who have sustained serious injuries with personalised rehabilitation, coordinated support, practical guidance and clear communication at every stage.

Our role is to guide individuals and families through complex situations with clarity, reassurance and compassion — making sure they access the right rehabilitation, treatment and support when they need it most.

We work closely with solicitors and insurers to coordinate the right support, ensure timely access to intervention and keep rehabilitation moving forward. Every decision is grounded in clinical reasoning and focused on the individual’s best interests.

Facing a life-changing injury is complex. Individuals and their families are often met with unfamiliar decisions to be made at a time of significant uncertainty.

Our case management services provide structured, person-centred support from the earliest stages of injury through to long-term rehabilitation and daily living. We work across multitrack, catastrophic and clinical negligence cases, supporting adults and children with complex injuries such as traumatic brain injury, spinal cord injury, amputation, complex orthopaedic trauma and complex mental health.

Our case managers are experienced clinicians who guide and advocate for individuals through each stage of their rehabilitation. We focus on independence, meaningful progress and long-term stability. We do this with purpose, compassion and clinical leadership – always advocating for the individual and supporting our partners with clear, timely information.

Immediate Needs Assessment (INA)

Early intervention has a proven impact on long-term health outcomes. That’s why we act quickly. Our Immediate Needs Assessments (INAs) provide a clear, structured understanding of an individual’s needs and priorities within days of instruction. The INA takes place in person, and our case managers will:

Engage with individual and their family

Understand what their life was like prior to injury, including work, social activities and their role within their family and the wider community.

Gain an understanding of case history

Review medical records and expert reports to fully understand the nature of the individual’s injuries and any prior treatment they have had.

Take a holistic approach to assessment

Take a holistic view, by assessing the individual’s physical function, social wellbeing and psychological and cognitive health.

Prioritise needs and determine actions

Identify the immediate priorities for rehabilitation and recommend next steps to increase independence as quickly and as fully as possible.

Create a bespoke rehabilitation plan

Set short and long-term rehabilitation goals based on what’s important to the individual to maintain their sense of identity.  

Comprehensive support tailored to individual needs

Our case managers provide consistent clinical oversight and coordination from the earliest stage of injury, adapting support as needs and priorities evolve.

Flexible support built around the individual and their family

Depending on need, case management may include:

  • Tailored therapy and targeted clinical intervention
  • Health and social care support

  • Help to stay connected with community access and social networks

Ongoing case management provides consistent guidance, coordination and clinical oversight. This includes:

  • Setting and reviewing personal rehabilitation goals
  • Planning and coordinating rehabilitation and wider support
  • Adapting interventions accordingly
  • Liaising with solicitors, insurers, and healthcare professionals across statutory and private services
  • Monitoring outcomes, risks and costs
  • Responding quickly and proportionately to change.

 

We provide support throughout litigation and post-settlement, ensuring the individual’s best interests remain the priority at every stage.
