Our role is to guide individuals and families through complex situations with clarity, assurance and compassion – ensuring they access effective rehabilitation, treatment and support at the moments they most need it.

With specialist clinical expertise across statutory and private services, and established relationships with a wide range of healthcare professionals and litigation stakeholders, our case managers coordinate interventions that are effective, clinically justified and tailored to the individual.

From the outset, our case managers consider both short and long-term goals and begin shaping a bespoke rehabilitation programme that reflects the individual’s needs, priorities and aspirations. We advocate for the client and provide consistent guidance, ensure clear communication, bring all stakeholders together and adapt support to ensure progress remains on track.

As an individual’s needs, priorities and circumstances evolve, ongoing case management provides clinical expertise, consistency, and proactive support during litigation and post-settlement. We support people of all ages living with catastrophic injury and complex needs to identify what’s important to them and focus on goals that reflect each person’s life and priorities, and work towards outcomes that strengthen independence, confidence and quality of life.

Our care service is shaped around preferences, lifestyle and the outcomes that matter most to the individual. From occasional daily assistance to dedicated 24-hour support teams, care is delivered through CQC-registered providers, ILS Case Management and CA Case Management, both part of HCML.
Care built around the individual, not a package
Every care arrangement supports the individual with the way they want to live, the routines they value and helping them maintain a sense of identity. Our fully managed, person-centred approach supports people from occasional daily assistance to dedicated 24-hour support teams, shaped entirely around the individual.
The right people, chosen with care
Our case managers lead recruitment, training and ongoing supervision of each care team, to ensure every support worker is the right fit clinically and personally. Engaging with individuals, their families and support networks is central to how we work.
